3. Transmission: The returns made under sections 45 and 46 of the Public Health Act (chapter S-2.2) must be signed and mailed in conformity with section 48 of that Act to the Institut de la statistique du Québec.
The return shall be made in the Forms annexed to this Regulation and the persons completing it must give all information required in the said forms.
